Insurance Information

I strongly encourage you to understand your health insurance coverage. Though I bill insurance for services, patients are financially responsible for copays, coinsurance, deductibles and non-covered services. Because plans vary widely and to avoid surprises, I recommend you contact your insurance company and gather some information about your policy and coverage. For your convenience, you may print this Health Insurance Verification Form and use it as a guide when calling your insurance company. Please BRING THIS FORM to your initial visit or turn in digitally with your other paperwork.

If your insurance company, policy or coverage changes, it’s always a good idea to recheck your benefits as they may change.

Car Accident?

You have the right to healthcare if you’ve been involved in an auto accident whether you were the driver, passenger, a cyclist or pedestrian. Don’t let insurance concerns prevent you from getting treatment. Oregon is a Personal Injury Protection state, which means your or another driver’s car insurance includes personal injury protection. This covers treatment for up to two years from the date of your accident and your benefits will cover evaluation and treatment at my office.

Medicare

Starting in 2020 there are many Medicare Advantage plans covering medical services provided by NDs. These may include Regence MedAdvantage HMO and PPO, Pacific Source MedAdvantage- Essentials HMO and Explorer PPO and MODA MedAdvantage.

Other than these plans, due to federal regulations, I do not participate in Medicare, Tricare, or Veteran’s Administration (VA) plans. Click here to get involved with including naturopathic medicine in Medicare.

Medicaid – Oregon Health Plan

I am sorry, but I am unable to accept Oregon Health Plan patients at this time. If you have a referral to me for care, please contact your referring provider to have the referral transferred to another provider.

Workers Compensation

Workers compensation policies vary and I generally do not accept workers compensation cases. I do make an exception for individuals who have a referral for acupuncture from a medical provider serving as their primary for the workers compensation case. If you are seeking care for a workers compensation case, please schedule a complimentary consultation with me first.

High Deductible? No Insurance? Discounts available.

Patients with insurance plans with high deductibles have the option for their insurance to be billed for services or not. Most often, it is recommended that patients with high deductibles do have their insurance billed. The “patient responsibility” for services may be about the same or less than our discounted fees and your payments are applied to your deductible. However, if you prefer not to have your insurance billed, this is an option and other discount options as described below are available. Note that if you opt out of insurance billing, the charges not submitted to insurance are not and cannot later be applied to your deductible.

For patients with no health insurance coverage, payment in full is due at the time of service. When paid in full, I offer a 20% discount on most services. For acupuncture services, multi-treatment packages are available – please inquire for details.
